Job Scope:
Programme driver for the eg.: Chronic Disease Self Management Programme (CDSMP) and other patient programmes in Polyclinic.
Programme marketing: Recruit and brief patients on the CDSMP and other patient empowerment programmes to instil health self-management skills in patients. This included the Watch Our Weight programme targeted at obese patients.
Project management: Organize, conduct and evaluate CDSMP workshops.
Patient engagement post chronic disease workshop.
Follow up on patients' status and behaviour post workshop.
Conduct support group meetings for patients who have undergone the CDSMP.
Participate and provide support for community outreach programmes.
Requirements:
GCE O/A Level, Diploma holders
Prior healthcare experience
Able to engage and relate/ counsel/ motivate patients
Good communications skills
Positive attitude and independent
Location: Central
Salary: Negotiable
Office hour, 5.5 days/week
